Background
Every winter there is a rise in healthcare demand in the community as well as hospitals, with high-risk populations being more vulnerable during this period. Since 2019, COORDINARE has offered funding opportunities to support practices lead heightened quality care for patients identified as high risk of being unstable, very unwell or admitted to hospital during Winter.
Building on this work, COORDINARE has commissioned a provider to deliver a place-based care coordination program for high-risk populations. The Care Coordination Program will operate regionally working alongside engaged general practice, in the community by supporting access to care, practice attendance, delivering home visits and telehealth support.
Expression of Interest (EOI)
COORDINARE is proposing to commission general practices to embed the Care Coordination Program and relevant care monitoring platform into their practice workflow. Lessons learned through embedding the Care Coordination Program will be used to develop practice workflows which inform and support a broader rollout of the program into primary care across the South Eastern NSW PHN catchment.
General practices in the area who have an interest in or who have participated in the shared care platform trial are welcome to apply for this project. Practices must have the capacity to interrogate their data to identify eligible patients and be willing to implement project activities over a 6 month project period from July 2022.
